Statewide In-Home Respite Care for Indiana Families
Supporting Dementia Caregivers Through Medicare GUIDE & PathWays for Aging

The Guiding an Improved Dementia Experience (GUIDE) Model was launched by the Centers for Medicare & Medicaid Services (CMS) in July 2024 as an eight-year, nationwide test program. It was created to close a long-standing gap: families living with a dementia diagnosis are often left to coordinate medical care, safety, and their own well-being largely on their own.

GUIDE organizes participating healthcare providers around one goal — helping people with dementia remain safely in their own homes and communities for as long as possible, while easing the burden carried by unpaid, family caregivers.

It does this through a dedicated care navigator, caregiver education, a 24/7 support line, and funded respite care, all built around a personalized dementia care plan for each family.

Support Built Around The Whole Family,
Not Just The Diagnosis

Dementia Care Navigator

A dedicated navigator helps coordinate medical and non-medical care and connects your family to community resources — transportation, meals, safety programs, and more.

24/7 Support Line

Access to a support line any time of day, so families aren't left managing a crisis or a hard moment alone.

Caregiver Training & Education

Practical guidance for unpaid caregivers on managing stress, improving home safety, and understanding how dementia will change day-to-day care over time.

Respite Care

Medicare-covered respite hours give family caregivers real breaks, which research links to caregivers being able to keep their loved one at home longer.

A Personalized Care Plan

Each person with dementia gets a care plan tailored to their diagnosis, stage, and household — not a one-size-fits-all checklist.

A Yearly Home Care Allowance

An annual allowance is available to help pay for in-home support, easing the financial pressure of keeping a loved one safely at home.

Turning Memory Care Support
Into Everyday Care at Home

GUIDE opens the door to navigation, training, and respite funding — but families still need trustworthy hands-on help to make that support real, day to day. That's where we come in.

As a non-medical home care provider in Indianapolis, we work alongside your care navigator and family to put GUIDE's respite hours and home care allowance to use, so your loved one is cared for and you get real time back.

We help you use your respite hours. We schedule trained caregivers during the breaks GUIDE makes available, so care never lapses while you rest.

We support the care plan your navigator builds. Our caregivers follow the routines, safety notes, and preferences already set for your loved one.

We provide consistent, dementia-aware caregivers. The same familiar faces help reduce confusion and anxiety for someone living with dementia.

We keep your family in the loop. Regular updates mean you always know how your loved one's day went, even when you weren't there.

We help you start the conversation. Not yet connected with a GUIDE navigator? We can help point you toward participating providers in our area.
