GUIDE Program Support
The GUIDE (Guiding an Improved Dementia Experience) program is a Medicare initiative designed to provide additional support for individuals living with dementia and their families.
What GUIDE Provides
- A dedicated dementia care navigator to coordinate care and connect families with community resources, including transportation, meals, and safety programs
- Training and support for caregivers to help manage stress, improve safety, and delay placement
- Medicare-covered respite care hours
- A yearly allowance to support in-home care so patients can remain safely and comfortably at home
Eligibility
Eligible patients may include individuals who:
- Have Traditional Medicare
- Have a suspected or confirmed dementia diagnosis (Alzheimer’s disease, Lewy Body Dementia, Frontotemporal Dementia, Vascular Dementia, etc.)
- Live at home or in the community, including Assisted Living Facilities (ALF) and Independent Living Facilities (ILF)
Not Eligible
The GUIDE program is not available for individuals who:
- Are enrolled in SNP (Special Needs Plan) programs
- Have Medicare Advantage plans
- Live in a long-term care (nursing home) setting
- Participate in Hospice or PACE programs
